Security Breach Notification
One of the vendors Shoreline Orthopaedics works with (TriZetto Provider Solutions) was impacted by a security incident, which affected some of our patients’ or their primary insured’s health information. Notices to affected individuals will be mailed starting February 6, 2026 from TriZetto Provider Solutions/Kroll Notification Navigator.

- The affected data varies by individual and may have included the following for patients and primary insureds: name, address, date of birth, SSN, health insurance member number/Medicare beneficiary identifier, health insurer name, primary insured or dependent information, and other demographic, health and health insurance information
- How will individuals be notified?
- TriZetto with the help of Kroll Notification Navigator will send letters to affected individuals to notify them of the breach on or after February 6, 2026
- Was any financial information accessed?
- Per TriZetto’s report, the breach did not include any payment information such as bank account/card or financial information
- Was any involved individual’s information involved in identity theft or fraud?
- Currently, TriZetto is not aware of any identity theft or fraud related to the use of any affected individual’s information
